I don't know who's giving out licenses in the Mushroom King, but they certainly seem easy to get! Dr. Baby Wario is the next to join Dr. Mario World, and he's coming on June 26th, 2020. Not sure I trust the guy, but I guess we'll all find out about his credentials later this week.
